Kist Hidatik Şüpheli Hastaların Tanısında ELISA ve İmmunokromotografik Yöntemin Karşılaştırılması

Özet

Kistik ekinokokkozis (KE), E. granulosus larvalarının etken olduğu, dünyada ve ülkemizde görülen önemli, zoonotik bir parazit enfeksiyonudur. Parazit karaciğer ve akciğer başta olmak üzere insanda pek çok organa yerleşebilmektedir. Tanıda ve tedavi sonrası takipte özellikle görüntüleme yöntemleri ve klinik bulguların yetersiz olduğu vakalarda seroloji oldukça değerlidir. Çalışmamızda Atatürk Üniversitesi Tıp Fakültesi Tıbbi Mikrobiyoloji Anabilim Dalı Laboratuvarına Kist Hidatik şüpheli hastalardan gönderilen serum örneklerinde anti- Echinococcus IgG antikorlarının ELISA ve İmmunokromatografik yöntemlerle araştırılması amaçlandı. Atatürk Üniversitesi Tıp Fakültesi Tıbbi Mikrobiyoloji Anabilim Dalı Laboratuvarına farklı kliniklerden gönderilen Kist Hidatik şüpheli 120 hastaya ait kan örnekleri çalışma kapsamına alındı. Hastalara ait serum örneklerinde anti-Echinococcus IgG antikorlarının araştırılması için ELISA yöntemi ile İmmunokromatografik yöntem kit üretici firmaların tavsiye ve önerilerine uyularak yapıldı. Çalışma yaptığımız 120 hastanın 32'si (%26.6) her iki yöntemden en az biriyle pozitif bulundu. Hasta örneklerinin 88'i her iki yöntemle negatif iken 25 örnek iki yöntem ile de pozitif bulundu. ELISA yöntemi ile pozitif bulunan 4 örnek İmmunokromatografik yöntemle negatif idi. İmmunokromatografik yöntemle pozitif olarak değerlendirilen 3 örnek ELISA yöntemiyle negatif bulundu. ELISA yöntemi sonuçları baz alınarak İmmunokromatografik yöntemin duyarlılığı %86.2, özgüllüğü %96.7 ve doğruluk derecesi %94.2 olarak hesaplandı. İmmunokromatografik yöntem test sonuçları ile ELISA yöntem test sonuçları birbiriyle uyumlu bulunmuştur. İmmunokromatografik yöntem çalışma kolaylığı sağlaması, hızlı sonuç vermesi ve düşük maliyeti nedeniyle diğer serolojik yöntemlerle kombine bir şekilde kullanılabilir.

Abstract

Cystic Echinococcosis (CE), which is caused by E. granulosus larvae, is one of the most important zoonotic parasitic infections in our country and all over the world. The parasite can settle in many human organs especially liver and lung. Serology is a valuable method when the clinical symptoms and screening methods are insufficient after the treatment. In this study, our aim was to investigate the anti-Echinococcus IgG antibodies by ELISA and Immunochromatographic methods in serum samples of cystic hydatid-suspected patients. A total 120 blood samples of cystic hydatid-suspected patients, which were sent to Atatürk University, Faculty of Medicine, Medical Microbiology Laboratory from various clinics, were involved in this study. Anti-Echinococcus IgG antibodies were investigated by ELISA and Immunochromatographic methods in serum samples of the patients by following the manufacturers’ advice and suggestions.

Total 32 (26.6%) patients in 120 was found as positive with at least one of the two methods. 88 of patients’ samples were found as negative, whereas 25 of total patients’ samples were positive with both two methods. Four samples, which were positive by ELISA, were detected negative with Immunochromatographic method. The sensitivity, specificity and degree of accuracy of Immunochromatographic method were 86.2%, 96.7% and 94.2% respectively based on the results of ELISA method. As a conclusion, it was found that the test results from ELISA and Immunochromatographic methods were compatible. Immunochromatographic methods may be used in combination with other serological methods owing to its quick result, low-cost and ease of operation options.
